Icing, De-icing Failures Blame Brazil Crash
Investigators concluded that the Voepass ATR 72-500 crash near Vinhedo on August 9, 2024, was the result of a deadly combination of severe icing, recurring de-icing system failures, and pilot distractions, set against a Voepass culture of normalizing deviations that left preexisting mechanical issues unaddressed. The final Cenipa report cites multiple contributing factors, including crew actions, weather conditions, and regulatory oversight, and urges changes to de-icing procedures in collaboration with EASA and ATR to prevent similar tragedies.
