Health insurers deploy tools to deny care

Last summer, the Trump administration, physician advocates, public health academics and activists urged commercial health insurers to streamline the prior authorization process, and major companies signed a six-part pledge to reduce it. Critics contend that prior authorization is just one of many denial-of-care tools that create a float—delaying payments and letting insurers invest premium cash for profit, often at the expense of patients.
